I was able to change one icon once, then when I didn't like the icon, have been unable to change it since. The other bookmark I wanted to update had no icon, and I have not been able to attach one.

Chrome remembers icon for a specified URI. When you force him to learn an icon for a bookmark (specified URI) you cannot force him to learn another, new icon for the same bookmark. That's just Chrome… The workaround for this is to change a little the URI of the bookmark, like e.g. add anchor ("#") at the end or add a meaningless parameter ("?parameter=nothing" or "¶meter=nothing" if already there some) – that way this will be completely different URI for Chrome (thus allowing it to learn new icon) while you will still open the same webpage.
Can you specify the URI of the another bookmarks, that didn't work? Sorry for late answer, probably you don't remember. Well, I use this extension with no errors since always…
