Add flag to indicate instructions have unexpected bits

The field constraints from Dismantle can specify "Quasi" constraints on bits, which are expected values but not strictly necessary from the perspective of decoding. If these bits have unexpected values, this should cause some kind of flag to be raised by the semantics indicating the erroneous state.
