DUO term request: IRB approval required and ERB approval required

Hi, the IRB approval required comes up in NCPI (NIH Cloud Platform Interoperability) and AnVIL consent information UI design. It will be helpful to create these two terms as the children terms of "ethics approval required" (DUO_0000021)
