Changes to CDB_TableMetadata relation trigger no invalidation of responses affected by it. The surrogate key for such responses is set correctly but nothing ever sends an invalidation for those responses.

The idea is to place a single cache-invalidating trigger on CDB_TableMetadata (#197)
and have it also invalidate the cdb_tablemetadata X-Cache-Channel
