[Multiple Ordering Providers] Integration into Test Card Creation

Why should we do this?

• Currently, users are unable to select the appropriate ordering provider when creating a new test card in SimpleReport.
• Integrating the ordering provider selection functionality into the test card creation workflow will ensure that the correct ordering provider information is captured and associated with each test record.

User story

As a SimpleReport user, I want to be able to select the appropriate ordering provider when creating a new test card so that the correct ordering provider information is associated with the test record.

Action Requested

1. Integrate the ordering provider selection functionality into the existing test card creation workflow.
2. Ensure the correct ordering provider information is captured and associated with the test record, including:
   • Displaying the list of available ordering providers for the user's facility
   • Allowing the user to select the appropriate ordering provider
   • Correctly storing the selected ordering provider information with the test record

Acceptance Criteria

Design

• Design is ready for handoff (i.e. dev ready)
• Design includes latest content updates
• Capture questions that come up during the content and design work

Implementation

• Integrate the ordering provider selection UI into the test card creation workflow
• Ensure the selected ordering provider information is correctly captured and associated with the test record
• Validate the integration works as expected through comprehensive testing

Review

• Reviewed with design and/or design lead
• Reviewed with product and engineering team
