[Python] Issues with AAD token refresh #1055

A couple of problematic token refresh behaviours that need to be addressed:

Tokens with a 'refresh_token' aren't being automatically refreshed as expected.
Tokens provided from external means (e.g. ADAL) lib aren't supported correctly (e.g. include different field capitalization like refreshToken).
Tokens that expire during Azure polled operations have no means by which the can be refreshed.
