The Problem:
The Berserker NPC's Retribution optional trait states "Whenever the Berserker takes damage, their next attack deals +2/+3/+4 bonus damage. This bonus stacks up to +8. Bonuses are lost when the Berserker attacks, or at the end of their next turn." I would confer with Tom to make absolutely certain, but I believe that for total wording consistency this should most likely specify that the bonus damage is on hit. The Berserker has no Reliable weapons and no bonus damage application in the game functions on a miss, so the inference is there, but nonetheless this may be considered wording worth amending to be completely clear if it is intended to only proc on hit.
